New Patient Paperwork
This is the paperwork you will need to fill out prior to your first exam so we can better understand your health and determine the most appropriate treatment methods.

This is the form you will use for children 8 and under.
Nutritional Symptom Analysis
This optional form is for patients interested in nutritional counseling, vitamins, or supplements.
Toxicity Questionnaire
We offer a 21 day detoxification program through Standard Process supplements. If you think you may be interested in this program taking this questionnaire is a good place to start! It is recommended that those who score higher than a 40 on this questionnaire participate in the cleanse program.
